There are no copies of tax forms available at the Provo City Library.

Taxpayers may also order current and prior year tax forms, instructions and publications by calling 1-800-TAX-FORM (1-800-829-3676).
Utah State
Beginning in 2012, the Utah Tax Commission is no longer producing or mailing paper versions of the TC-40 Individual Income Tax booklet and forms. Because of this, there are no copies of tax forms available at the Provo City Library.  Taxpayers may order current and prior year tax forms, instructions and publications by calling 1-800-662-4335, ext. 6700 or by emailing taxforms@utah.gov.

Where can I get help with my taxes?

Income Tax Assistance for the Elderly

Free Income tax assistance for those ages 65+ are available by appointment through the AARP Tax Center.  Call 801-900-3386 for more information.

Utah Tax Help and VITA

VITA offers free assistance with simple, current-year tax returns to low-income, elderly, non-English speaking and physically challenged taxpayers who cannot afford professional tax help. Please check the Tax Help Utah website for more information or call 211.

All Taxpayer Assistance Centers (TACs) are closed due to COVID-19. 
When calling to schedule an appointment, customer service representatives are available Monday through Friday, 7 a.m. to 7 p.m.
