Where do I get Tax forms?

The IRS is encouraging people to file tax forms electronically. To learn more about e-filing your State and Federal income taxes, see the Electronically Filing Tax Forms section below.
Taxpayers may also order current and prior year tax forms, instructions and publications by calling 1-800-TAX-FORM (1-800-829-3676).
There are no copies of tax forms available at the Provo City Library.
Utah State
Beginning in 2012, the Utah Tax Commission is no longer producing or mailing paper versions of the TC-40 Individual Income Tax booklet and forms. Because of this, there are no copies of tax forms available at the Provo City Library.

Income Tax Assistance for the Elderly

Free Income tax assistance for the elderly will be available by appointment on select dates at the Provo Recreation Center.

Utah Tax Help and VITA

VITA offers free assistance with simple, current-year tax returns to low-income, elderly, non-English speaking and physically challenged taxpayers who cannot afford professional tax help. Please check the Utah Tax Help website for more information or call 211.
