Steps to Obtaining a Provo City Library Card

All Provo and Orem City residents are entitled to a
Provo City Library Card.

Non-residents may obtain a Provo City Library Card
by paying a fee of $50 for six months
or $100 for one year of library use.

Library Cards for Provo City Educators

STEP 1

Complete a Provo City Library Card Application. You can download the Card Application as an Adobe Acrobat PDF file. To view or print a PDF file, you need Adobe Reader. You can download a free copy of the Reader from the Adobe web site. 

STEP 2

Provide BOTH of the following:

1)       Photo Identification – Picture ID issued by government, school or employer

2)       Proof of Permanent Address – A document bearing the applicant’s name and current address.  (Cannot be photo ID)

    Examples of Proof of Address:
  • Rent receipt/contract
  • Mail postmarked within the last 3 months
  • Utility bill
  • Preprinted checks

  STEP 3

Present all information at the Check-out Desk.

Note:  Children under the age of 18 may obtain their own library card.  An application is not required, however, the parent/guardian must be present and in possession of a Provo City Library Card.  Minor’s cards are issued attached to a parent/guardian.


Library Cardholders are Responsible for:

  • All Materials, fines, and fees charged to their library card, regardless of who uses it, until it is reported lost or stolen.
  • All transactions on any card belonging to persons for whom they are legally responsible.
  • All losses and/or damages to library materials checked out on this card.
  • Prompt payment of any charges incurred.
  • Prompt notification of any change of address.
  • Immediate notification of missing card.
For Your Protection:
  • Never loan your card.
  • Report missing card immediately by calling 801-852-6660

 

Pasos para obtener una tarjeta de la biblioteca dela ciudad de Provo

Todos los residentes de Provo tienen derecho a obtener una tarjeta de la biblioteca. 
La tarjeta es gratuita para residentes de Provo y Orem.

Personas que no son residentes de Provo pueden obtener una tarjeta de la biblioteca pagando una cuota de $50.00 por seis meses o $100.00 por un año.

PASO 1

Se puede descargar la solicitud como archivo PDF.  Para ver o imprimir un archivo PDF, se necesita el programa Adobe Reader. Se puede descargar una copia gratis de Reader del sitio web de Adobe.

PASO 2

Proveer cada una de las siguientes:

1.  Identificación con foto – La identificación debe ser publicada por una escuela, empleador o del gobierno.

2.  Verificación de su dirección permanente – Un documento con el nombre del solicitante de la tarjeta y la dirección actual. (No puede ser la identificación de foto.)

Ejemplos de verificación de dirección:

            Recibo de renta o contrato de renta
            Correspondencia marcada por el correo dentro de los últimos 3 meses
            Factura de servicios públicos de agua, luz, gas, etc.      
            Cheque impreso con su nombre y dirección

PASO 3

Seleccione un diseño de tarjeta y presente toda la información al Escritorio de Préstamos.

Nota:  Niños menores de 18 años de edad pueden obtener su propia tarjeta de biblioteca.  Una solicitud no es requerida pero los padres o guardiánes legales deben estar presente y deben tener una tarjeta de la biblioteca.  La tarjeta para un niño menor de edad estará unida a la tarjeta del padre o guardián legal.
